Patients are asked how likely they are to recommend their GP practice. Practices submit monthly to CQRS; NHS England publishes the results plus a submission checker flagging data that is missing, nil or unchanged.
Nothing outside the practice checks the numbers. A practice that collected 200 responses including 50 poor ones can submit 200 "very good" and the publication carries it unchallenged. A high score is evidence about what was submitted; a low score is often a collection problem rather than an unhappy list.
The percentage is taken over rated responses, so 12 responses and 1,200 can show the same score. Always read it with responses per 1,000 patients and the margin of error shown on the score.
"No submission", "submitted zero responses" and "breakdown withheld for small numbers" are distinct facts and shown separately. FFT was also suspended for COVID, so the series jumps Feb 2020 → Jul 2022 and trend lines break across the gap.
